Since 2019, Communities of Care (CoC), a five-year initiative that supports collaborative approaches to wellbeing in the greater Houston Metropolitan Area, has partnered with local leaders, residents, and organizations to address the systemic drivers of health inequities and work toward sustainable, community-driven change.
Through initiatives like the DREAM 77021 Collaborative, we’ve empowered residents in neighborhoods such as OST/South Union in Houston to advocate for their wellbeing, taking on challenges like environmental justice, safety, and neighborhood infrastructure.
CoC, in partnership with local organizations and community leaders, provides tools, training, and support to help residents become active agents of change in their communities. From addressing inequitable disciplinary practices in schools to advocating for safer streets, our work reflects our commitment to creating safer, healthier, and more connected communities.
