In 2011, Prevention Institute created the Community-Centered Health Homes (CCHH) model to provide healthcare organizations with a roadmap for identifying local community determinants of health and collaborating with partner organizations to improve community conditions. Since the model was introduced, it’s been piloted in health clinics in Louisiana, Florida, Alabama, Mississippi, North Carolina, and Texas to take on issues as diverse as food insecurity, poor housing, workplace environmental hazards, and a lack of neighborhood parks and playgrounds.
