Larry Cohen, PI’s founder, will present during ASTHO’s Integration Forum call on July 25 with all partners for a discussion on bridging clinical care and community action through the implementation of the Prevention Institute's Community-Centered Health Homes (CCHH) model.
The Integration Forum (formerly known as the ASTHO-Supported Primary Care and Public Health Collaborative) is a partnership of more than 50 organizations and 100 individual partners seeking to inform, align, and support the implementation of integrated efforts that improve population health and lower healthcare costs.
